A completed "Fire Training Authorization form"  must be submitted to the instructor for the following courses: Firefighter 1, Firefighter Survival, FAST, Truck Company, Mask Confidence, Hazmat Technician, Advance Hazmat Technician, Confined Space Rescue, and Passenger Train Emergency Rescue Procedures. This is a type-able form, fill in, print, and give to the instructor on the first night of the class.

A completed "Residential Registration Form" is also required for courses given at the New York State Academy of Fire Sciences in Montour Falls, NY. This is a type-able form that can be printed and faxed to the academy.

Use the "Fire Academy Bookstore Order Form" to place orders for books and other materials for courses from the Academy Book Store. Note: The Academy Bookstore discounts Delmar/Thompson books 25% off list. List price is $77.95, the NYS Fire Academy bookstore price is $58.46, ISBN number is 1401835759.

If you choose not to purchase your own Delmar textbook you may borrow one from your instructor. Select the appropriate borrowing agreement for each course, print it out, and present the completed agreement to the State Fire Instructor. You are responsible for the care and to return the textbook either when you complete the course or leave the prior to the end of the course.

NOTE: Students who lost or destroyed their borrowed textbook can either buy a replacement and give it to the State Fire Instructor or make a check payable to "NYS Academy Fire Science" in the amount of $77.95 and give it to the State Fire Instructor.
